Describe yourself...tell us about the things you're involved in...the things that make you unique...the words that your friends use to describe you...any extra curricular activities that you belong to. Basically...we just want to know a little bit about who you are because that will help us to capture who you are and determine if you will be a good fit for what we're looking for. *
If selected...you will be photographed during the shooting portion of the workshop. There will be 8-10 photographers at the photo shoot working with the senior models.

You will be receiving the edited digital files (.jpg images) from the photographers approximately 1 month after the workshop. There are no guarantees as to how many images you will receive, but you should be able to expect to receive at a minimum 8-10 images from the workshop attendees.

By selecting YES below you are agreeing to receive the digital negatives as your compensation for your participation and also agreeing that you will use the images for your own personal use...and not for monetary gain of any kind. You are also agreeing to allow the photographers in attendance to use the images taken of you to market and promote their own photography studios. *
I agree to arrive at the shooting location no later than 4:15pm on Friday, September 2nd. The shoot will go until approximately 7:15pm.

The shoot will be held in Scottsdale, AZ. A detailed map will be emailed to you if you are selected to participate. *
I understand that if I am selected, I will be modeling outdoors and will be given dress recommendations and ideas on what to bring/wear for the photo session...and I will do my best to stay within the clothing suggestions when choosing my styles and looks for the day. *
Please upload up to 3 images of yourself (.JPG FORMAT ONLY). We are not making our selections based on appearance alone...however we are looking to include a variety of looks within the senior modeling group and will be making our selections in order to accomplish that goal.

Applications received without a picture will not be considered.

MODEL RELEASE ::
I hereby grant Sandi Bradshaw Photography / Avenue 17, Sandi Bradshaw and her legal representatives and assigns, the irrevocable right to use and publish photographs of me (if 18 or over) my child, or children for the purpose of display, editorial work, trade, portfolio, advertising, website, or publication to promote the Sandi Bradshaw Photography / Avenue 17 studio or Sandi Bradshaw. I agree that these images can be used by Sandi Bradshaw Photography / Avenue 17 without additional compensation to the above stated. * I agree I do not agree

Reproduction includes, but is not limited to scanning, downloading, right-clicking, emailing, saving to harddrive, posting on websites or blogs, photocopying, obtaining screen shots, or printing in any manner. * I agree to honor the copyright laws as stated in this agreement.
